dc.description.abstractGelasticepilepsyorlaughingseizureshavebeenhistoricallyrelatedtochildrenwithhypothalamichamartomas. Wereportthreeadultpatientswhohadgelasticepilepsy,definedasthepresenceofseizureswithaprominent laughcomponent,includingbrainimaging,surface/invasiveelectroencephalography,positronemissiontomography,andmedical/surgicaloutcomes.Noneofthepatientshadhamartomaorotherhypothalamiclesion.Two patientswereclassifiedashavingrefractoryepilepsy(onehadbiopsy-provenneurocysticercosisandtheother onehippocampalsclerosisandtemporalcorticaldysplasia).ThethirdpatienthadnolesiononMRIandhad completecontrolwithcarbamazepine.Bothlesionalpatientsunderwentresectivesurgery,onewithcomplete seizurecontrolandtheotheronewithpooroutcome.Althoughhypothalamichamartomasshouldalways beruledoutinpatientswithgelasticepilepsy, laughingseizurescanalsoarisefromfrontalandtemporal lobefoci,whichcanbesurgicallyremoved. Inaddition,wepresentthefirstcaseofgelasticepilepsydueto neurocysticercosis.
