Study design: Retrospective, cohort study of infants <32 weeks’ GA born at centers of NEOCOSUR Network between January 2015 and December 2020. Subjects were categorized as exposed vs non-exposed to antenatal MgSO4. Primary outcomes were death, incidence of severe IVH (Grade III-IV) and severe IVH/death. Secondary outcomes included relevant morbidities. Results: 7418 VLBW infants were eligible. Antenatal MgSO4 was associated with a significantly decreased death rate after admission (aOR 0.67 [95% CI, 0.49–0.94]) and severe IVH/ death (aOR 0.68 [95% CI, 0.50–0.93]). No significant reduction in severe IVH was observed (aOR 1.11 [95% CI, 0.72–1.71]). No differences between groups were observed in rates of morbidities. Conclusion: Antenatal MgSO4 was associated with a decreased death rate after admission and in severe IVH/ death.
- ItemASSOCIATION OF SUBCLINICAL ATHEROSCLEROSIS WITH DIFFERENT STANDARDS OF THE METABOLIC SYNDROME AND INSULIN RESISTANCE(2013) Arnai, P.; Dominguez, Angélica; Barja Y., Salesa; Godoy, I.; Villarroel del Pino, Luis A.; Castillo Valenzuela, Oscar; Farías Jofré, Marcelo; Mardones, FranciscoBackground and objectives: we have previously selected two metabolic syndrome (MS) components blood pressure (BP) and high density lipoprotein cholesterol (HDLC) as associated with carotid intima media thickness (CIMT) a surrogate marker of subclinical atherosclerosis in children (Rev Med Chile 2012; 140: 1268-75). We aimed to ascertain with a higher sample size the influence of the above mentioned variables and three standards defining the MS plus three standards defining insulin resistance (IR). Methods: A cross-sectional study of 447 children 10-14 years old of low socio-economic strata from an urban area of Chile was performed during years 2009-2011. This sample was selected considering the presence of one or more MS component and IR. Anthropometry and BP were assessed. A blood sample for determination of glycaemia, insulinemia (quimioluminiscence) and blood lipids were taken; HOMA was calculated and three standards were applied to select HOMA-IR cases. Three standards to define MS were also used. Medium CIMT was assessed using ultrasonography with automated software. Chi-squared test and stepwise regression were used. Results: Mean age was 11.5 ±1.0 years old; 59% girls; 92.6% pubertal; 72% excess weight. Three standards for MS had prevalence of 24.4% (Cook, 2004), 14.1% (IDF, 2007), 42.9% (De Ferranti, 2004). Three standards for HOMA-IR had the following prevalence: 15.2% (Barja, 2011), 37.4% (Burrows, 2006), 23.0% (De Onis, 2012). Elevated blood pressure and reduced CHDL had significant associations with CIMT ≥ percentile 75. The logistic regression for CIMT ≥ percentile 75 only selected BP ≥ percentile 90 (OR = 2.963) and CHDL < 40 mg/dL (OR = 1.789) Conclusions: None of the various classifications for the MS and the HOMA-IR were selected in the multivariate analysis confirming the previously found influence of increased BP and reduced CHDL on medium CIMT.

Aim: To assess the efficacy ofi a tele-care self-management support model (ATAS) on metabolic control of patients with DM2 attending primary care centers in a low income area in Santiago, Chile. Material and Methods: Two primary care centers were randomly assigned to continue with usual care (control group, CG) or to receive additionally 6 telecare self-management support interventions (IG) during a 15 month period. Glycosylated hemoglobin (HbA1c) was used to measure metabolic control of DM2; the "Summary of Diabetes Self-care Activities Measure" and the "Spanish Diabetes Self-efficacy" scale were used to measure self-management and self efficacy, respectively. Changes in the use of health services were also evaluated. Results: The IG maintained its HbA1c level (baseline and final levels of 8.3 ± 2.3 % and 8.5 ± 2.2% respectively) whereas it deteriorated in the CG (baseline and final levels of 7.4 ± 2.3 and 8.8 ± 2.3 % respectively, p < 0.001). The perception of self-efficacy in the IG improved while remaining unchanged in the CG (p < 0.001). Adherence to medication, physical activity and foot care did not change in either group. In the IG, compliance to clinic visits increased while emergency care visits decreased. Conclusions: The ATAS intervention, in low income primary care centers, significantly increased the probability of stabilizing the metabolic control of patients with DM2 and improved their use of health services.

Aim: To esta blish a model to predict the quality of sexual life in hysterectomized women, six months after surgery. Material and Methods: Analytical, longitudinal and prospective study of 90 hysterectomized women aged 45±7 years. Two structured interviews at the time of surgery and six months later were carried out to determine the characteristics of sexuality and communication within the couple. Results: In the two interviews, communication and the quality of sexual life were described as “good” in 72 and 77% of women, respectively (NS). The variables that had a 40% influence on the quaity of sexual life sixth months after surgery, were oophorectomy status, the presence of orgasm, the characteristics of communication and the basal sexuality with the couple. Conclusions: The sexuality of the hysterectomized women will depend, on a great extent, of pre-surgical variables. Therefore, it is important to consider these variables for the education of hysterectomized women
