- ItemDiagnóstico precoz de la enfermedad de Parkinson(Sociedad Iberoamericana de Información Científica (SIIC),, 2009) Juri Clavería, Carlos Andrés; Uribe San Martin, Reinaldo MoisésParkinson's disease (PD) is a neurodegenerative disorder clinically characterised by motor symptoms, and these manifestations arise after an extensive loss of dopaminergic neurons in the substantia nigra. In the last decades evidence has emerged to show that the onset of neurodegeneration in PD probably begins many years before the onset of motor symptoms. Additionally nonmotor manifestations, such as olfactory dysfunction, REM behavioural disorder, depression and constipation, also arise earlier than motor manifestations in the course of the disease. Neuroimages are useful in identifying subjects at risk of developing PD, as the result of early non-motor manifestations, or carriers of genetic mutations associated with familial PD. In this article we review the available evidence in diagnosing PD in the early non-motor stage and how this approach could help us to improve the treatment of PD patients

J. van; Bassett, Anne S.; Boot, ErikBackground: 22q11.2 deletion syndrome (22q11.2DS) has been associated with increased risk of early-onset Parkinson's disease (PD). Objective: To determine the prevalence and predictors of PD in a large international 22q11.2DS sample. Methods: The sample comprised 856 adults (median age 28 (range 16-76) years; 53.0% female). PD was defined as clinical diagnosis by a neurologist (including bradykinesia, rest tremor and/or rigidity). Age-specific risk and predictors of PD were analyzed using Kaplan-Meier curve and Cox regression. Results: PD was present in 1.8% (95% CI: 0.9-2.6%) of the sample, 3.4% (95% CI: 2.2-4.6%) when including uncertain PD (clinical diagnosis or suspicion, but not meeting all criteria), and 14.0% (95% CI: 6.9-21.0%) of those aged >= 50 years. Median age at motor onset was 45 (range 20-66) years. None of the factors considered were associated with PD. Conclusions: Given high PD prevalence and young onset, we propose periodic motor evaluations from age 40 years in 22q11.2DS.

Caso clínico(2010) Aguilar Quezada, Enrique Alfonso; Pollak Wegner, Débora Berta; Necochea Krause, María Cecilia; Fadic Ruiz, Ricardo Julio Nicolas; Juri Clavería, Carlos AndrésParkinson disease (PD) is a movement disorder characterized clinically by the variable combination of rigidity, bradykinesia, rest tremor and postural instability. Usually postural instability is a late-onset manifestation and is frequently associated with axial manifestations and with a poor prognosis. We report a 67-year-old female with orthostatic tremor as the etiology of her postural instability. The patient was treated with increasing doses of clonazepam, reaching 2 mg/day, and levodopa. There was an improvement of postural instability with a good response of parkinsonian symptoms.
